I do include all of the dev files.  It is just a single RPM. 
Basically when you do the "make install" part after you compile the 
source code (PHP Source, NOT ClamAV Lib), all of it gets dumped.

What you can try though is this;

/usr/local/bin/phpize

Notice I added the "local" part.  That is where my PHP gets installed. 
Give that a try!  You still do need (which you installed) the GCC 
Tools to run the ./configure & make though!
