This appears to be a BQ issue.

I have many copies of phpBB running, both phpBB2 and phpBB3
All are updated to latest version

Some require Admin to authorize registrations
phpBB2 > General Configuration > Enable account activation > Admin
phpBB3 > User registration settings > Account activation > By Admin

This works OK on all older installs with SMTP or local mail.

Now I'm seeing installs to new vsites on same machines not working with Admin activation?

phpBB2 ERROR
Ran into problems sending Mail. Response: 503 5.0.0 Need RCPT (recipient)
DEBUG MODE

User gets email, but admin email appears to bounce?

Does anyone know the fix for this?
I seem to have misplaced my notes.
