Ok, I figured it out.  I was so convinced the problem was with cmu itself that I didn't really bother trying to analyze the error message in cmu.log.  (Thanks Stephanie for at least making me focus there)

The problem was simply a bad access.  When I tried makemap hash access> access I got an error about duplicte lines.  So when I removed the duplicate lines and ran makemap, everything worked fine.
