IMHO
Generally there are 2 approaches
Top-down or
Bottom-up
1) For top-down, Document for all codes. eg: that php file do what
Dont need to be too technical. Simple explanation will do.
Once all codes are documented, a schematic can be formed
and program flow can be studied, modified and maintained
2) For Bottom-up, One or more individuals have an idea
for change. Implement the idea and document the changes
later.
Anyway, times is cost. And project process is heavily depended on how many resources are dedicated.
ps: Anyone knows of any Open-source "all-in-one" (DFD-codes-document-Specification) project management
